Release of Liability & Indemnification Agreement

I hereby release Kennedale Community Theatre in the Park and volunteers (jointly hereinafter referred to as “The Theatre”), of and from any and all claims, demands, and causes of action for any damages, injuries, and expenses arising out of or in any way connected with MY CHILD’s participation in any activity. This release and indemnification of The Theatre does not extend to or apply to gross or willful negligence with respect to an injury to MY CHILD. I understand that theatrical activities, etc. can be dangerous. As the Parent/Guardian I am responsible for evaluating MY CHILD’s fitness to participate in these activities and I am responsible for any and all insurance, including but not limited to, Medical Insurance, to cover MY CHILD’s participation in these activities. I hereby certify that MY CHILD is fit to participate in the activity covered by this Release and Indemnification Agreement.

I do hereby agree to indemnify and hold harmless The Theatre, of and from any and all claims, demands, and causes of action for any damages, injuries, and expenses arising out of or in any way connected with MY CHILD’s participation in any activity. I also do hereby agree to indemnify and hold harmless The Theatre, any physician, and theatre representative who provides any help, medical assistance, treatment, and/or care for MY CHILD of and from any and all claims, demands, and causes of action for any damages, injuries, and expenses arising out of or in any way connected with any help, medical assistance, treatment, care of, and medicine for MY CHILD.

I hereby request that MY CHILD be allowed to participate in Theatre sponsored activities. I understand that by MY CHILD participating I consent to the use of any videos, photographs, slides, audio recordings, or any other visual or audio reproduction in which my student may appear by Kennedale Community Theatre in the Park programs, Facebook Page, website, etc. I understand that these materials may be used for promotion of Kennedale Community Theatre in the Park, which may include enrollment and fundraising efforts.

By signing my signature on the registration form, I agree that the releases, the indemnity, and the other agreements contained in this document are valid and binding on me and are a part of the consideration for MY CHILD being allowed to participate.

Registration Payment Instructions

After completing you registration form above, please follow the link below to pay your registration fee. Please ensure that you list the names of all students you are enrolling in camp on the payment form so that KCTP can tie it to your registration form.

Please note that registration payments are non-refundable.
